A duplicate  driving licence will be issued in the following circumstances

    a. When the licence is lost or destroyed

    b. When the licence is defaced or torn or completely written up

    c. When the photograph affixed to the licence requires replacement

    Requirements

            1. Application in Form – D L L D
            2. Recent pass port size photographs – 2 Nos (duly attested)
            3. Receipt for payment of fees of Rs.200/-
            4. Original licence written of or defaced if available.
            5. Attested photocopies of DL if available in case of loss of licence.
